Femoroacetabular impingement (FAI) is a structural hip condition where abnormal bone shape at the femoral head (cam impingement), the acetabular rim (pincer impingement), or both causes friction and labral damage at the limits of hip movement. FAI predominantly affects younger, active adults (20s-40s) and is increasingly recognised as a significant cause of early hip degeneration if left untreated.
FAI Symptoms Relevant to Walking Aid Decisions
- Deep anterior groin pain at end-range hip flexion (sitting, squatting, pivoting)
- Pain during and after activities involving hip loading (walking hills, stairs, prolonged sitting)
- Clicking or catching sensation in the hip at end range
- Pain that is worse after activity and slow to settle (inflammatory component)
Does a Walking Cane Help FAI?
FAI pain is typically activity-related and position-related rather than constant weight-bearing pain. A walking cane provides moderate help:
- Reduces hip joint contact force during walking by 20-40% on the contralateral-cane-supported stride
- Allows a more even, deliberate gait that avoids the provocative hip positions
- Reduces fatigue from altered gait mechanics
The limitation is that FAI pain is predominantly at end-range hip flexion -- the positions during chair transfers and stair climbing, which a cane assists less directly than a frame. For flat-ground walking, a cane is often helpful during FAI flares.
