Walking Cane for Psoriatic Arthritis: Joint Protection, Dactylitis, and Enthesitis Management

Psoriatic arthritis (PsA) is an inflammatory arthritis associated with psoriasis, affecting approximately 30% of psoriasis patients. Unlike rheumatoid arthritis, PsA has a more variable pattern: it can be oligoarticular (a few joints) or polyarticular, and it produces unique features including dactylitis (sausage finger/toe) and enthesitis (inflammation at tendon and ligament insertion points). These features create specific walking challenges that standard cane guidance does not address.

Enthesitis and Walking: The Plantar Fascia and Achilles

Enthesitis in PsA commonly affects two walking-critical sites: the plantar fascia insertion at the heel, and the Achilles tendon insertion. This produces heel pain that can be severely limiting for walking. Unlike plantar fasciitis of mechanical origin, PsA enthesitis is driven by inflammation and does not respond as predictably to load modification alone.

A walking cane during enthesitis flares reduces the total lower limb loading per step, which reduces the repetitive insertion stress at the affected enthesis. For plantar/heel enthesitis specifically, a cane on the contralateral side reduces stance-phase force through the affected foot -- the same mechanism as for plantar fasciitis.

Dactylitis and Grip

Dactylitis (diffuse swelling of a whole finger or toe) in the hand affects cane grip. A swollen finger cannot bend comfortably into a grip position on a narrow handle, and any grip pressure on a dactylitic digit is painful. The options:

  • Broad-contact handle: the Anatomic Grip (1.9 N/cm2 peak, contoured contact) allows the hand to grip with less pressure per contact point than a T-bar. Dactylitic fingers rest against the contour with less active compression
  • Wrist lanyard: reduces the active grip force required by providing retention through the wrist loop -- the fingers do not need to grip as hard because the lanyard prevents the cane falling
  • Forearm platform crutch: when dactylitis is severe and grip is significantly limited, the platform distributes load through the forearm, bypassing the hand entirely

Variable Joint Pattern

PsA joint pattern is unpredictable -- different joints may be affected in different flares, and the asymmetry varies. Unlike OA where the worse joint is consistently on one side, PsA may produce bilateral knee involvement one month and unilateral ankle involvement the next. Cane placement must therefore be reassessed with each flare rather than fixed at a single side.

Skin and Handle Hygiene

Psoriasis on the palms (palmoplantar psoriasis) creates a specific consideration for handle materials. Psoriatic skin is more sensitive to friction and may react to certain materials. Natural wood handles are hypoallergenic and do not contain the synthetic materials (plastics, rubbers) that can irritate sensitised psoriatic skin on the palm. Metal handles may cause contact reactions in some psoriasis patients, particularly when sweating.

Biologic Therapy and Cane Use

Modern PsA management often involves biologic therapies (TNF inhibitors, IL-17 inhibitors, JAK inhibitors) that produce rapid and significant improvement in inflammation. As with other inflammatory conditions, cane use should be expected to reduce as biologics take effect (typically weeks to months). During dose adjustments, switches, or treatment breaks, symptoms return and cane use should be resumed without waiting until function is severely limited.
