Chronic ankle instability (CAI) from repeated ankle sprains that fail to heal adequately is treated surgically by lateral ligament reconstruction, most commonly using the modified Brostrom-Gould procedure. This technique repairs and reinforces the anterior talofibular ligament (ATFL) and calcaneofibular ligament (CFL) -- the two primary lateral ankle stabilisers most commonly disrupted in inversion ankle sprains. The post-operative protocol determines how long a walking cane is required and when it can be weaned.

Brostrom Repair: Standard Post-Operative Walking Aid Protocol

The Brostrom repair reconstructs the lateral ankle ligaments using the patient own tissue (direct repair, not graft). The repaired ligaments need protection from tension and stress for the first 6-12 weeks while healing occurs. Standard protocol:

  • Weeks 0-2: Non-weight-bearing in short leg cast; crutches mandatory
  • Weeks 2-6: Partial weight-bearing in CAM boot; transition from crutches to single cane during this period
  • Weeks 6-10: Full weight-bearing in boot or lace-up ankle brace; cane as needed for confidence and balance
  • Weeks 10-16: Transition to shoe; cane typically discontinued; physiotherapy for balance retraining (proprioception rehabilitation)

When a Cane Is Most Useful After Brostrom Repair

The period from weeks 2-6 is when the cane provides the most clinical value after Brostrom repair. The patient is moving from non-weight-bearing (where crutches are needed) to partial weight-bearing (where one crutch or a cane is sufficient). The cane provides:

  • Load reduction on the healing lateral ankle ligaments
  • Balance compensation during the period when the ankle proprioceptive system is impaired (both from the original CAI and from the surgical healing)
  • Confidence on uneven surfaces -- critical for preventing a fall that could disrupt the repair

Proprioception Rehabilitation and the Cane Wean

Ankle proprioception is impaired both by chronic ankle instability (the recurrent sprains damage the mechanoreceptors in the lateral ligaments) and by surgery (the repair involves reefing and tightening the capsule, temporarily altering mechanoreceptor function). Proprioception rehabilitation -- balance board, single-leg stance progressions, perturbation training -- should begin as the cane is weaned, to fill the proprioceptive gap the cane was compensating for.
