Walking Cane for Heart Failure: Cardiac Rehabilitation, Activity Pacing and Breathlessness

Walking Cane for Heart Failure: Cardiac Rehabilitation, Activity Pacing and Breathlessness

Heart failure (HF) is a clinical syndrome in which the heart cannot pump sufficient blood to meet the body metabolic needs, leading to reduced exercise tolerance, breathlessness, and fluid retention (oedema). Heart failure affects approximately 64 million people worldwide. Walking limitation in heart failure is multifactorial: reduced cardiac output limits oxygen delivery to muscles; peripheral oedema impairs limb mechanics; and skeletal muscle changes (cardiac cachexia) reduce muscle function independently. A walking cane in heart failure has a specific and often underappreciated role.

How Heart Failure Limits Walking

  • Reduced cardiac output: the heart cannot increase output adequately with exertion; oxygen delivery to muscles is insufficient; patients fatigue rapidly
  • Lower limb oedema: fluid accumulation in the legs increases weight, impairs muscle function, and reduces ankle mobility
  • Breathlessness: both pulmonary congestion (fluid in lungs) and low cardiac output cause dyspnoea on exertion
  • Cardiac cachexia: in advanced HF, muscle wasting reduces walking endurance and strength
  • Medication effects: beta-blockers limit heart rate response; diuretics may cause electrolyte disturbances affecting muscle function

Walking Cane for Heart Failure: Mechanism and Role

The cane reduces the cardiovascular demand of walking by reducing the mechanical work the legs must do per step, allowing a given cardiac output to sustain a longer walking distance. Key benefits:

  • Reduces peak oxygen consumption per metre of walking
  • Allows walking at a lower heart rate for a given distance (important with heart rate-limited cardiac output)
  • Reduces lower limb oedema impact by offloading some weight from the oedematous legs
  • Provides psychological confidence to maintain walking, counteracting deconditioning

Heart Failure Severity and Walking Aid

NYHA Class Symptom Level Walking Aid
NYHA Class I (no symptoms at ordinary activity) Symptoms only at very heavy exertion No aid needed for normal activity
NYHA Class II (slight limitation) Symptoms on climbing stairs or walking uphill Cane for hills and stairs; may not need on flat
NYHA Class III (marked limitation) Symptoms on minimal walking; less than 100m Cane for all walking; rollator for very limited patients
NYHA Class IV (symptoms at rest) Breathless at rest; minimal activity possible Rollator or wheelchair; focus on transfers and positioning

Cardiac Rehabilitation and Walking Cane

Cardiac rehabilitation (CR) after hospitalisation for heart failure includes supervised exercise training. Walking is the cornerstone exercise in CR programmes. A cane is not incompatible with CR participation -- many CR patients use walking aids during supervised exercise sessions. The cane allows patients who would otherwise be too limited to participate in CR to walk the required distances at suitable intensity.
