Walking Cane for Mixed Connective Tissue Disease (MCTD): Overlap Features and Mobility

Mixed connective tissue disease (MCTD) is a distinct overlap autoimmune syndrome characterised by high-titre anti-U1 RNP antibodies combined with features of at least two of: SLE, systemic sclerosis (SSc), inflammatory myopathy (polymyositis/dermatomyositis), and rheumatoid arthritis. MCTD is considered a true overlap syndrome rather than undifferentiated disease. The mobility impairments in MCTD reflect the combination of features from its component diseases, and walking cane selection must account for this complexity.

MCTD Features That Affect Mobility

  • Inflammatory myopathy component: proximal muscle weakness (hip girdle and shoulder girdle) is common in MCTD with myositis overlap. Ascending stairs and rising from chairs are disproportionately impaired.
  • Articular component (RA-like): symmetric small joint arthritis with larger joint involvement; knee and ankle synovitis causes antalgic gait
  • SSc component: fibrotic changes may affect pulmonary function (exertional breathlessness, reducing walking tolerance) and oesophageal motility; Raynaud phenomenon affecting hand grip
  • SLE component: fatigue, avascular necrosis (if steroid-treated), serositis causing pain with trunk movement
  • Pulmonary arterial hypertension (PAH): occurs in MCTD and causes exercise-limiting breathlessness

Cane Handle Selection for Raynaud in MCTD

Raynaud phenomenon in MCTD causes digital vasospasm triggered by cold. Metal cane handles (aluminium, stainless steel) become cold rapidly and can trigger Raynaud.
