Walking Cane for Seronegative Arthritis: HLA-B27 Conditions and Enthesitis

Seronegative arthritis refers to inflammatory arthritis in which rheumatoid factor (RF) is negative -- a heterogeneous group that includes ankylosing spondylitis (AS), psoriatic arthritis (PsA), reactive arthritis, and undifferentiated spondyloarthropathy (SpA). Many seronegative conditions are HLA-B27 associated and share the feature of enthesitis (inflammation at tendon and ligament insertions into bone) in addition to synovitis. Walking cane use in seronegative arthritis depends on which joints are primarily affected and the presence of enthesopathy at the lower limb.

Walking Cane for Ankylosing Spondylitis

AS predominantly affects the spine but hip involvement occurs in 25-35% of patients and is the most common reason for walking impairment. Hip flexion contracture from AS is progressive and difficult to treat medically. Walking cane use in AS hip disease follows standard hip OA cane principles (contralateral hand). Spinal rigidity in AS means the patient cannot lean forward compensatorily, making hip pain particularly disabling for walking. Biologics (TNF inhibitors, IL-17 inhibitors) are highly effective for AS activity -- cane need should be reassessed after starting biologic therapy.

Walking Cane for Enthesitis (Plantar Fascia and Achilles)

Enthesitis at the plantar fascia origin and Achilles insertion is characteristic of seronegative SpA. It causes heel pain that is sharply worsened by walking (first steps in the morning are typically worst). A walking cane reduces the ground reaction force transmitted through the heel enthesis during each step. This is particularly useful for managing enthesitis flares while systemic treatment (NSAIDs, biologics) takes effect.
