Walking Cane for Subtalar and Midfoot Arthritis: Hindfoot Coalition and Flatfoot Deformity

Walking Cane for Subtalar and Midfoot Arthritis: Hindfoot Coalition and Flatfoot Deformity

Subtalar arthritis (talocalcaneal joint), midfoot arthritis (tarsometatarsal joints, Lisfranc joint complex), and hindfoot coalition (tarsal coalition -- abnormal bony or cartilaginous fusion between hindfoot bones) are less common but significantly disabling causes of foot pain and gait impairment. These conditions are often overlooked as sources of walking difficulty because ankle X-rays are often normal -- the pathology lies in the hindfoot and midfoot rather than the tibiotalar joint.

Subtalar Arthritis and Gait

The subtalar joint (between talus and calcaneus) controls hindfoot inversion and eversion during gait, allowing the foot to adapt to uneven terrain. Subtalar arthritis causes:

  • Pain on uneven surfaces (where subtalar motion is greatest)
  • Stiffness of hindfoot inversion/eversion
  • Characteristic antalgic gait with avoidance of heel strike on the affected side
  • Difficulty walking on cobblestones, grass, or any non-level surface

Midfoot Arthritis (Lisfranc and Tarsometatarsal Joints)

Midfoot arthritis is often post-traumatic (Lisfranc injury is a common cause), or develops with flatfoot deformity as the midfoot collapses. Symptoms:

  • Pain at the midfoot with weight-bearing, particularly at push-off
  • Midfoot swelling and bony prominences
  • Difficulty wearing regular footwear

Hindfoot Coalition

Tarsal coalition (most commonly calcaneonavicular or talocalcaneal) causes a rigid, painful flatfoot. Pain is present on uneven ground. Coalition is typically diagnosed in adolescence but may not cause significant symptoms until adulthood when the coalition becomes rigid.

Walking Cane for Subtalar and Midfoot Conditions

Condition Gait Problem Cane Role
Subtalar arthritis Hindfoot pain on uneven terrain; stiff hindfoot Contralateral cane; reduces subtalar loading; particularly useful on uneven ground
Midfoot (Lisfranc) arthritis Push-off pain; midfoot instability Contralateral cane reduces push-off demand; custom rigid insole or rocker sole
Hindfoot coalition (rigid flatfoot) Rigid painful flatfoot; uneven ground avoidance Contralateral cane; custom orthotics; surgical coalition resection or fusion if severe
Post-subtalar fusion Eliminated subtalar motion; compensatory midfoot and ankle Cane for 8-12 weeks post-fusion; longer than ankle fusion typically
